Teva provides access to affordable dental benefits to help you and your family maintain your dental health—and your smile!

What to know

  • Delta Dental of New Jersey is the provider of our dental coverage.
  • If you enrolled in dental coverage for the first time for 2025, an ID card will be mailed to your home address on file. Be sure to show your ID cards to your providers so they have the information for their billing records.
  • Plan members can access Delta Dental’s MySmile platform to view their benefits and take advantage of easy-to-use tools.
  • Our dental plan provides an enhanced benefit for covered members (children and adults) with a qualifying special health care need. Plus, there is also additional support for those managing periodontal disease and extra covered cleanings for expectant mothers and those with heart disease or diabetes.
  • You can go to any dental care provider you choose; however, if you use an out-of-network dentist, you will be responsible for the annual deductible, coinsurance and any amounts above the reasonable and customary (R&C) charges in your area.

Vision

Teva provides vision benefits to help you and your family manage your annual eye care needs.

What to know

  • Aetna VisionSM Preferred is the provider of our vision coverage.
  • If you enrolled in vision coverage for the first time for 2025, an ID card will be mailed to your home address on file. Be sure to show your ID cards to your providers so they have the information for their billing records.
  • Teva offers two vision plans: the High Plan and the Low Plan.
  • The High Plan includes a $0 copay for routine, in-network exams, a variety of covered lenses for children and coverage for new frames every year.
  • The Low Plan includes lower employee costs per pay period, a higher cost for routine, in-network exams and lenses ($20 copay) and a lower allowance for contact lenses/frames.
  • Both plans feature special offers on contact lenses and other eyewear.
