Disability and leave benefits offer financial protection and/or job protection in case you are unable to work due to an illness, injury or approved leave of absence. See our Leave of Absence Guide for more details.

What to Know

  • Teva provides access to disability and leave benefits to to employees regularly scheduled to work at least 30 hours per week.
  • Disability and leave benefits give you and your family peace of mind and help protect your income and/or job in the event you are disabled and can’t work.
  • Our disability and leave benefits are administered by Matrix Absence Management.
  • To report a leave, visit Matrix or call (877) 202-0055.
  • Coverage provided at no cost to employees regularly scheduled to work at least 30 hours per week.
  • The plan pays 100% of your weekly salary, up to eight weeks, after a seven-day waiting period (there is no waiting period for Maternity Leave; STD benefits begin on the first day of leave) to protect your income for a non-work-related injury or illness.
  • After the waiting period, our vendor partner, Matrix, administers the coverage up until the 189th day of your disability (27 weeks).
  • If you continue to be unable to perform your job duties solely because of an injury or illness, the plan will pay 66.67% of your salary during weeks 10 through 26.
  • If approved by Reliance Standard (sister company to Matrix), Long-Term Disability (LTD) coverage begins at the end of STD coverage.
  • Coverage provided at no cost to employees regularly scheduled to work at least 30 hour per week.
  • Approximately six weeks prior to the end of the STD period, Matrix will determine if there is sufficient evidence to automatically bridge your claim to LTD. You do not have to submit any additional information to Matrix to initiate this review.
  • If approved by our vendor partner, Reliance Standard (sister company to Matrix), the plan pays 60% of your gross base earnings, up to a maximum monthly benefit of $15,000.
  • Coverage offered to employees grade 10 and above who are regularly scheduled to work at least 30 hour per week.
  • Supplemental Individual Disability Insurance (IDI) through Unum can supplement your existing basic Long-Term Disability (LTD) coverage offered at no cost to you.
  • If you qualify, the current LTD benefit begins when Short-Term Disability (STD) coverage ends. The LTD plan pays 60% of base salary, up to a reduced $15,000 per month.
  • This “buy-up” option allows for more income protection by providing a maximum benefit of 75% of base salary, target bonus and target commission, up to $15,000 in addition to the existing group LTD insurance you automatically receive from Teva.
  • This benefit is only offered once a year during the Annual Benefits Enrollment period each fall.

For short-term disability (STD) leave, some benefits continue while you’re on disability and some require you to make separate payments for continued coverage.

  • Medical, Dental and Vision: Regular payroll deductions will be deducted from the disability payments you receive from Matrix.
  • Life and Accidental Death & Dismemberment (AD&D): Teva pays the premium for coverage.
  • 401(k): Deductions will stop while on leave.
  • Spending Accounts: Deductions will stop while on leave.
  • Accident, Critical Illness, Hospital Indemnity, LifeLock or Whole Life with Long-Term Care (LTC): If enrolled, you will receive a letter from our benefits administrator, Winston, regarding payments.
  • Supplemental Individual Disability (IDI): If enrolled, you will receive a letter from Unum regarding payments.
